Duties of personnel when reporting an accident

Employees

  • Inform their supervisor as soon as possible after an accident/incident has occurred;
  • Sign and date the accident book as conformation that they agree the accident recorded is a true and accurate record.
  • Be prepared to assist the Safety Advisor’s investigation.

Supervisors

  • In the case of a serious accident/incident, inform the Safety Advisor as soon as possible.
  • Complete the appropriate Incident Report Form and send to the Consumer Services Department within seven days.
  • Be prepared to assist the Safety Advisor’s investigation.

Contract Managers

  • Sign and complete the Incident Report Form.

Safety Advisors

  • Check the accident book and Incident Report Form.
  • Where necessary, carry out an investigation.
  • Complete an F2508 if accident or dangerous occurrence is reportable under RIDDOR and inform the Health and Safety Executive.
  • Check the supervisor has completed an Incident Report Form.
